NOTES:

• The textures for the screen are not really needed unless your program requires actual texture files - otherwise they add no detail except for the color being slightly lighter than the black edges around it (which you could simply adjust yourself).
• The UV of the screen is stretched to perfectly fit the 1:1 UV space - to add your own image/video file, you must manually adjust the image or the UVs until it looks right.
