Original Size you will recieve is 2048 x 2048 and includes a bump map if needed.
In 3D Studio Max you just add the black and white image to the bump mapping slot in your material editor for this particular material object. Lots of tutorials online on how to do this.

I've already made this tileable in Photoshop CS5 so no editing is required in it's current state. Could be used for game content, terrains, etc.
